Please fill out the form below, or email ctl@uwo.ca with the details of your consultation request. Appointments are generally booked 2-3 weeks in advance. If you have not received a response to your inquiry within five business days, please email ctl@uwo.ca. We will be checking and responding to emails during regular office hours: Monday to Friday from 8:30am-4:30pm.
